CONSUMER HEALTH DATA PRIVACY NOTICE

This notice explains how Legacy Hair by Kaur (Kaur Lounge Inc.), 2124 James Street, Bellingham, WA 98225, handles "consumer health data" as defined by Washington's My Health My Data Act. It supplements our Privacy Policy.
1. WHAT HEALTH DATA WE COLLECT
During consultations, booking, and services, we may collect:
- Information about hair loss, thinning, or scalp conditions
- Allergies, skin or scalp sensitivities, and reactions to products
- Pregnancy or medical conditions or medications you tell us about because they may affect your service
- Photos of your hair and scalp, taken with your permission
2. WHERE IT COMES FROM
Directly from you: in person, by phone, by text or email, or through our website and booking forms.
3. WHY WE COLLECT AND USE IT
Only to provide the services you request safely, such as choosing the right products, recommending patch tests, planning hair loss and extension solutions, and keeping notes for your future visits.
4. WHO WE SHARE IT WITH
We share consumer health data only with service providers that store records for us, such as our booking platform (Boulevard), and only so they can provide that service to us. We do not sell consumer health data, and we do not share it for advertising.
5. YOUR RIGHTS
You have the right to:
- Confirm whether we collect, share, or sell your consumer health data, and see that data
- Get a list of the third parties and affiliates we have shared it with
- Withdraw your consent
- Have your consumer health data deleted

If we decline your request, you may appeal by replying to our response with "Appeal" in the subject line. If you are not satisfied with the outcome of your appeal, you may contact the Washington State Attorney General at atg.wa.gov.
